Transport of Calcium in Renal Tubule:
Only ionized calcium, and its complexes with small anions, are filtered at the glomerulus. The proximal tubule reabsorbs 65% of total filtered calcium. This occurs primarily by paracellular route. The driving forces are the lumen positive electrical gradient, generated by chloride… reabsorption, and the concentration gradient, generated due to water reabsorption. The thick ascending limb of the loop of Henle reabsorbs 25%. Here also, the reabsorption occurs mainly by paracellular route. The driving force is luminal electro-positivity, generated by the diffusion of potassium into the lumen, and, diffusion of chloride, into the interstitium. The distal convoluted tubule reabsorbs 8% of total filtered calcium. And this occurs by transcellular route, involving epithelial calcium channels TRPV 5&6 at the apical membrane, and, calcium ATPase pump, and sodium-calcium-exchanger, at the basolateral membrane. Parathyroid hormone, and vitamin D, increase calcium reabsorption by transcellular route. An increase in plasma concentration decreases the reabsorption.
